• An islet cell tumor producing VIP (vasoactive intestinal polypeptides) results in a syndrome with massive,
watery diarrhea, high serum calcium and low serum potassium , chloride and blood sugar (hypoglycemia).
VIPomas are characterized by secretory diarrhea (pancreatic cholera), which persists even when fasting (this
tumor is also referred to as WDHA—watery diarrhea, hypokalemia achlorhydria). With severe diarrhea, up to
1 1/2 gallons (6 L) of small bowel fluid can be lost each day. Persistent diarrhea during fasting is very
suspicious for VIPomas.
• Tumors that overproduce somatostatin cause electrolyte (mineral) abnormalities, diabetes mellitus, weight
loss, malabsorption of food in the intestine (steatorrhea), loss of gastric acid (gastric achlorhydria) and
occasionally gallstones.
